In the Government order 1st read above, the Government have constituted a Committee to study the functioning and procedures followed in the A.P.G.L.I. Department and to make recommendation for simplification of the same to submit Proposal Forms.
2. The Committee in the reference 2nd read above, has submitted a report to Government making certain recommendations for effective functioning of the department, recommending not to insist the Proposal Form for additional policies, since the employees have already furnished certain information in the Proposal Forms submitted towards fresh insurance.
3. The Director of Insurance, Hyderabad in his letter 3rd cited has submitted the proposals on the recommendations of the Committee and stated that he may be permitted not to insist for the proposal forms for issue of additional policies to the extent of compulsory slab and the Drawing & Disbursing Officers may be instructed to submit the information with regard to the recovery of further premium details, such as Name of the Employees, Designation, Father’s Name, Date of Birth, Policy Number, Basic Pay, amount of premium increased and month and year in which the increase is effected, so that the Director of Insurance may prepare the additional insurance policies and issue to the concerned through respective Drawing & Disbursing Officers,. In respect of fresh insurance and in case where premium is deducted more than compulsory slab, the submission of proposal forms are compulsory.

After careful examination of the proposal of the Director of Insurance and accepting the recommendation of the committee, Government hereby order that there is no necessity for submission of proposal forms for additional policies here after to the extent of compulsory slab. In respect of fresh insurance and in the cases where the premium is more than compulsory slab, the submission of proposal form to the respective District Insurance Office is compulsory for issue of policies.
5. All the Drawing and Disbursing Officers are therefore requested to furnish the information to the concerned District Insurance Officers showing the details such a, Name of the Employee, Designation, Father’s Name, Date of Birth, Policy Number, Basic Pay, amount of premium increased and month and year in which the increase is effected, after recovery of increased premium from the salary of the employees without fail.
They are also solely responsible for obtaining additional policy to the extent of premium recovered and to mentioned the correct Policy Numbers in the monthly schedules.
